This notice explains how your health information may be used and shared, and your rights. I follow federal (HIPAA) and Texas privacy laws to protect your information. Please review it carefully.

How I use and share your information

I may use or disclose your Protected Health Information (PHI) for:

Treatment
To provide therapy services and maintain your clinical record.

Payment
To bill and receive payment from you or your insurance (may include diagnosis and service details).

Operations
For scheduling, documentation, and basic business functions.

Electronic systems and telehealth

I use:

  • Sessions Health (client portal & records)

  • Zoom (telehealth sessions)

These platforms are designed to be secure, but no system is completely risk-free. By using them, you acknowledge and accept these risks.

When your authorization may be required

I will get your written permission before:

  • Sharing psychotherapy notes (in most cases)

  • Disclosures not listed in this notice

You may revoke authorization at any time in writing.

When disclosure may be required by law

I may share information without your permission if required, including:

  • Risk of serious harm to you or others

  • Suspected abuse or neglect (child, elderly, or disabled persons)

  • Court orders or legal proceedings

  • Licensing board or government oversight

Your rights

You have the right to:

  • Access or request a copy of your records

  • Request corrections

  • Request limits on how information is shared

  • Request confidential communication methods

  • Receive a copy of this notice

My responsibilities

I am required to:

  • Keep your information private

  • Follow this notice

  • Notify you of a data breach

Certain records related to substance use treatment may be subject to additional federal protections. When applicable, those records will be handled in accordance with federal law.

Changes to this notice

I may update this notice at any time. The current version will always be available on my website.

Texas privacy

Texas law may provide additional protections for mental health records, which I follow.
